Your First Visit

Your first visit is focused on understanding your story, your goals, and what matters most to you. We will discuss your medical history, symptoms, lifestyle, and any concerns you have.

I will complete a thorough assessment looking at strength, flexibility, posture, breathing patterns, movement, coordination, and other factors that may be contributing to your symptoms.

We will create a personalized plan together, begin working toward your goals during your first visit, and you will leave with clear next steps to continue your progress.

Pricing & Payment

Bloom Pelvic Health & Physical Therapy is a cash-based practice, meaning payment is collected directly from the patient at the time of service rather than submitted directly to insurance. This allows for individualized care with the time and attention needed to focus on your goals.

Initial Evaluation: $150
Follow-Up Visits: $120

Some insurance plans offer out-of-network benefits that may provide reimbursement for physical therapy services. If applicable, an itemized receipt can be provided upon request. Because benefits vary by plan, we recommend contacting your insurance provider directly for information about your coverage.

Do I Need a Referral?

No referral is needed to begin physical therapy at Bloom. Oklahoma allows direct access to physical therapy care, meaning you can start treatment without first seeing a physician.

If you do have a referral from your healthcare provider, bringing a copy to your first visit can be helpful. If continued care beyond the direct access period requires a physician referral, I will work with you to obtain one.

How Long Are Visits?

Your initial evaluation is typically 75–90 minutes, and follow-up visits are typically 45–60 minutes depending on your specific needs.

The length of care varies based on your goals, symptoms, and progress. We will discuss your individualized plan after your evaluation.

What Should I Wear?

Wear comfortable clothing that allows you to move easily and participate in your assessment and treatment.
